HOW TO COMPLETE ONLINE REGISTRATION & SUBMIT DOCUMENTS

STEP 1
 
  • Click on this LINK to complete the online registration.
    • Be sure to select the correct school year. 
    • Complete all screens to enroll student. Note: student must live within Wagon Wheel boundaries to attend Wagon Wheel otherwise you will need to request a School of Choice from your home school.
  • When complete, print registration form (only 1 copy is needed), sign, and date.
 
STEP 2
     
*** Scan the following documents to glcornish@capousd.org:***
 
  • Immunizations  - Student's complete immunization record.  Please note that all required immunizations must be up to date. See below for immunization requirements.
     * Polio – 4 doses (3 doses ok if one was given ON or AFTER 4th birthday)
     * DTAP – 5 doses (4 doses ok if one was given ON or AFTER 4th birthday)
     * MMR – 2 doses (both doses need to have been given ON or AFTER 1st birthday)
     * Hep B – 3 doses
     * Varicella – 2 doses (new requirement as of July 1st, 2019)   
           
WE CANNOT REGISTER ANY STUDENT WITHOUT COMPLETE IMMUNIZATIONS.
 
  • Online Registration Form - signed and dated
  • Recent mortgage statement OR lease agreement.  (***Note:  If you are purchasing (or have purchased) a new home and you do not have a mortgage statement, please provide 1st page of escrow instructions or final closing statement if escrow has closed.  If you own the property, please provide a copy of the 1st page of the recorded Grant Deed.
  • Recent utility bill (cable bills are not acceptable)
  • Home Language Survey
  • Enrollment Information Form
  • Indian Student Eligibility Certification Form (Only if Applicable)
  • Help Us Get to Know Your Child (TK & Kindergarten Only)
  • Report of Health Examination for School Entry is required by State law for all 1st grade students                                                                                     Physicals given for Kindergarten entry will be accepted if completed no sooner than 18 months prior to 1st grade entry.
  • Oral Health Assessment Form (to be completed during student's kindergarten year)
     
 
STEP 3
 
Once you have scanned all documents in Step 2, call the office to confirm receipt.  Bring the following documents IN PERSON to finalize registration
  • Birth Certificate
  • Driver's license (1 parent only)
 
****ALL OF THE DOCUMENTS ABOVE MUST BE RECEIVED IN ORDER FOR YOUR CHILD TO COMPLETE THE REGISTRATION PROCESS AND FOR STUDENT TO PHYSICALLY ATTEND SCHOOL ON THE 1ST DAY.***
